You will no longer have to wait months for follow-ups, sending messages through the patient portal is made easy, and resources are made available to improve your knowledge and empower you to live better, healthier and happier.

Why is a membership-tier model better for my health?
Bright $400
All patients start with a Bright beginning. It is 4 month-long commitment at $399/month, or $1500 paid-in-full for 4 months, during this exciting time, we are going to make the most appropriate adjustment to your health to influence food, mood, sleep and energy. We believe this is the most critical time of our partnership. We investigate your particular health story. You will learn about your own body responses to your changes. As we adjust and discover your strength, we can tailor a treatment plan that will bring closer to your goals.
Month #1: Intake with Dr Trieu.
Month #2: Initial nutrition consult with support team member.
Month #3: Follow-up with Dr. Trieu, reviewing labs orders, reviewing target and reassessing progress or change in direction.
Month#4: Support appointment with support team member.

The Freedom membership is the reason we started this journey. You have been so far as to no longer be dependent on supplements, restrictive transitional diets, and your lifestyle choices are such that you do not need your doctor’s active support, although we will be there if needed.
Check-in once a year with Dr. Trieu. Follow-up visit only if needed to transfer to Star membership. Continue to access patient portal messaging, discounted labs and supplements, and office hours, group visits.

Policies- Why we do not bill insurance
We are passionate about the kind of care that functional medicine is able to deliver to our patients. We love seeing the impact we have in their lives as our mission is to deliver superior and personalized health care. Although we realize that not billing the insurance may present a financial challenge for patients, we are choosing to do so to allow us to deliver care that goes beyond what is normal and acceptable to the insurance standard. We work on personalized changes that address the root cause of your symptoms, including nutritional and lifestyle recommendations, so that you can live, feel and be a better version of yourself. This model of optimization of health takes time.
Unfortunately, the traditional model of healthcare supports 15 minutes appointments, with an army of support staff working behind the scene, and doctors are not reimbursed for spending time with patients or creating better health in the long term. Insurance companies are invested in a traditional model favoring short-term results, with prescriptions, procedures, and/or surgeries, and do not meet the health need of the patients who need to address the root cause of their chronic symptoms.
Superbill Information-Only for non-membership fees
On request, we will provide you with a Superbill, which is an itemized receipt with the ICD-10 codes and CPT codes used for the visit, you can submit to your insurance provided that you have out-of-network benefits. You can call the number on the back of your insurance card, the member’s benefit number and inquire about their reimbursement.
The CPT code used at the initial visit is 99205, and for follow-ups 99215, 99214. Virtual visit codes are 99423 and 99424.

Cancellations
Please provide at least 48-hour notice (2 days) for all cancelled or rescheduled appointment. By doing so, we can ensure that we be mindful of the need of other patients who are waiting for appointments.

How much do labs cost?
Conventional labs (Labcorp, Quest) can bill the insurance directly. We also have negotiated Labcorp cash prices for patient who choose not to use their insurance. Make sure you use in-network labs when billing through your insurance.
Functional Medicine labs are usually cash-paid. However, the specialty labs can bill insurance whenever possible. Costs are transparent and there is no up-marking for the labs. We discuss the lab pricing at your visit so you can plan accordingly as they would require an investment on your part.
Specialized lab tests to be considered depending on your own journey are comprehensive stool tests, nutrient testing, organic acid testing, cyclic hormone testing, food sensitivities, mold/mycotoxins, and heavy metals. We use Genova, Precision Analytical/DUTCH, Diagnostic Solutions (GI Map), ZRT, and Doctor’s Data, etc.
